Salary: £22,401 pro rata (1.0 FTE); actual salary is £14,530 (actual salary)

Location: Multi-sited with requirement for frequent travel across Cwm Taf

Hours of work: 24 hours per week (3 posts)

Type of contract: Fixed term from November 2018 to 30 April 2021

Description

This an exciting opportunity to work for Merthyr and the Valleys local Mind on a major research project  to test the effectiveness of a new social prescribing service for mental health.  At present, many people go to their GP for help with problems caused by difficult social, economic or environmental issues that can’t fully be improved by available clinical interventions.  We aim to help these people get the right support by linking them with the community resources that can help, such as welfare services, wellbeing activities or peer support groups.

The Link Worker will have a central role to play in the success of this service.  They will meet with clients referred by primary care, and they will help them to access a range of services and support available in local communities. The focus will be on empowering the client to make their own choices and to set personal goals.

The Link Worker will need to build good relationships with their local primary care teams, as well as develop an in-depth knowledge of local services and activities.  As this is a research project, the Link Worker will support the collection of client information so that we can evaluate the impact of the service.

The Link Worker will join the project during the planning phase and will have the opportunity to shape the development of the service, and to work with a team of colleagues from other pilot sites across Wales.  The ideal candidate will be passionate about helping people, they will know their local area, and they will be reliable and organised.
